• Certificate of Live Birth -  Raised seal and/or County Clerk stamp must be evident. 

• Residency proofs - One proof MUST be a utility bill, property tax document or lease. 

• Parent/Guardian Identification - Acceptable ID includes drivers license, State ID or passport.

• Immunization Record - Must be signed by an medical provider or local health department.    

• Other documents that may be required:

Custody/Guardianship court document

Individualized Education Program (IEP) or 504 Plan

Kindergarten/Young 5s/1st graders:  Hearing, Vision, Dental assessments

Grades 6-8: Middle School registration 

Grades 9-12: High School Course Request  

Grades 10-12: Transcript from the previous school/s attended

NOTE: Once registration is submitted, additional documentation cannot be uploaded.  Please provide via fax, email or in person.

>  PDFs upload the best, sometimes they need to be compressed. Photos, especially in HEIC format, aren't always readable.
